Office Policies
The fee is payable at the beginning of the session unless other arrangements have been made. Payment of psychotherapy related fees will be made to Kenneth J. Fallin, Ph.D., LCSW by check, cash, credit or debit card. Payment of nutrition and metabolic measurement related fees will be made to Carrie Lutter by cash or check. At this time, all transactions are fee for service and insurance is not accepted.
Please make a note of my cancellation and payment policies. Your appointment is a reservation of our time together, whether you choose to use it or not. If you must cancel or change an appointment, call as soon as possible. Giving advance notice allows me to fill that time with a patient from the waiting list. For an appointment missed or cancelled less than 24 hours in advance, my policy is to charge the full appointment fee. This fee is not reimbursed by insurance.
Everything you tell me is confidential. If it is necessary for me to speak with someone else about you, I will do so only if I have your written permission. If you were referred by your doctor, he/she will receive a report from me within 15 days of your first visit.
